To assist you in filling out the new high technology company quote form, we suggest you print this page before proceeding. To print this page, click on the Print Button on your Browser's Toolbar. You can also print by clicking on the File Menu and then selecting the Print option.

TO FILL OUT THE FORM

The form is four (4) pages long. You can click on each blank in the form to enter text or you can press the "Tab" key on your keyboard to move between the blanks of the form.

After you have filled out the form fully, please click on the "Submit Form" button (The "Submit Form" button is located on the bottom of the last page.) and your email program should open up with a new message with your form data as an attachment. Simply send the message to instantaneously send your form to our specialized representatives.

TO PRINT A COPY OF YOUR COMPLETED QUOTE FORM

1. After you have completed the quote form and
BEFORE
you submit the form to our respresentatives, click on the Print button on the Adobe Acrobat Reader toolbar. The Toolbar will be located at the top of the Adobe window. Diagram A below displays a portion of the Adobe Acrobat Reader toolbar. The Print button is marked with a RED arrow.

TO SAVE A COPY OF THE QUOTE FORM TO FILL OUT AT A LATER TIME

1. To save a blank form on your computer to fill out at a later time
, click on the Save button on the Adobe Acrobat Reader toolbar. The Toolbar will be located at the top of the Adobe window. Diagram B below displays a portion of the Adobe Acrobat Reader toolbar. The Save button is marked with a BLUE arrow.

2. A "Save As" dialog box will appear on your screen. Select the location where you want to save the form and type in the filename where prompted (be sure to maintain the ".pdf" extension). While we suggest you save the quote form on your desktop for ease of use, you can save the quote form anywhere you'd like on your computer.

3. When you are ready to fill out the saved quote form, connect to the Internet, open the form, fill it out and then press the "Submit Form" button. Your email program should open up with a new message with your form data as an attachment. Simply send the message to instantaneously send your form to our specialized representatives.
